MEDICAL HOME CONCEPT...

The Medical Home Project is a system of linkage between participating physicians, who donate appointment slots for uninsured children from low-income families, and referring nurses.  Children seen through the Medical Home Project do not qualify for (or are in the qualifying process) for KidsCare or AHCCCS.

The AzAAP Medical Home Project has provided more than 13,000 services for 6,300 children since it began in August 1993.  Services include primary care and specialty referrals, prescription medication, diagnostic laboratory services and eyeglasses.
